4.Terms of Agreement and Rights of Cancellation

Payment Authorization:

I authorized Yada Wear to withdraw payment of at least $15.20 and up to $17.60 (up to 20% discount) plus S/H from my credit card as specified here in this Agreement for the order of the T-SHIRT OF THE MONTH. I give my permission that, if Yada Wear is unable to collect payment in full from my credit card account, then Yada Wear may make multiple attempts to collect for up to 60 days, and, no more than twice to add $1.00 to my balance to cover the ongoing costs for administration of the collection of my account. I agree to pay a $10.00 service fee in the event a charge is dishonored for any reason. I will hold Yada Wear harmless for all special or consequential damages, whether direct or indirect, resulting from any wrongful debit to my account.

Term:

The term of this Agreement is one year. This Agreement will be automatically renewed annually on each anniversary date of this Agreement and will remain in effect until (1) I elect to cancel this Agreement with Yada Wear T-SHIRT OF THE MONTH by written notice mailed to Yada Wear 216 Lemmon Drive #209 Reno, NV 89506 and received by the 20th of the month prio to shipment. (2) Yada Wear T-SHIRT OF THE MONTH cancels this Agreement upon written notice to me at my address set forth herein, which I agree Yada Wear T-SHIRT OF THE MONTH may do at any time in its discretion.

Satisfaction Guarantee:

If I am not completely satisfied with any T-Shirt I purchased from Yada Wear T-SHIRT OF THE MONTH due to a defect of the material, I may return the item, together with a copy of the invoice to Yada Wear 216 Lemmon Drive #209 Reno, NV 89506 within 20 days of purchase and Yada Wear will (1) credit my account for the total purchase price of the item (less shipping and handling charges), (2) upon my written request, replace the item without charge, or (3) upon my written request, refund the total purchase price of the product (less shipping and handling charges) which in turn will cancel my agreement with T-SHIRT OF THE MONTH.

Email and Telephone Communication:

I consent to Yada Wear sending me email messages or notifying me by telephone regarding its products, services, business opportunity and other topics that Yada Wear considers may be of interest or benefit to me. I agree that my receipt of any telephone, e-mail or other contacts by Yada Wear shall not be deemed a violation by Yada Wear of any state or federal telephone solicitation, no-call, anti-spam, customer protection or similar laws.

Personal Information:

I consent to the collection and lawful use by Yada Wear of the personal information contained on this form and information regarding my purchases from Yada Wear. I also consent to the disclosure of such information and information regarding my purchases from Yada Wear to all of Yada Wear management team.

Amendments:

I understand that by written notification from Yada Wear sent to me in my order or by some other means Yada Wear may amend this Customer Agreement. I agree to be bound by all such amendments and that my only remedy for not accepting any such amendments is to immediately terminate this Agreement.

Cancellation Rights:

I may cancel this Agreement for any reason at any time by giving written notice to Yada Wear 216 Lemmon Drive #209 Reno, NV 89506 bearing my original signature, printed name, address, and Customer Number. Written cancellations received by Yada Wear on or before the 20th of the month will be effective the month received, written cancellations received by Yada Wear after the 20th of the month will be effective the following month. In addition to the foregoing, I may cancel this agreement for any reason within three business days from the date hereof by giving written notice to Yada Wear bearing the date, my printed name, and my original signature.

BY SIGNING, I ACKNOWLEDGE THAT I AM AT LEAST 18 YEARS OLD AND HVE CAREFULLY READ AND AGREE TO THE YADA WEAR T-SHIRT OF THE MONTH CUSTOMER MEMBERSHIP AGREEMENT TERMS AND CONDITIONS OF THIS AGREEMENT.
